Your HGV medical and D2 and D4 forms
D2 is your application. D4 is your medical report. They do different jobs. Here is how to prepare for this early stage of a Great Britain licence application.
Reviewed 21 October 2026. Licensing guidance here concerns Great Britain. Check Northern Ireland requirements separately.
What are the forms for?
The D2 form asks DVLA to add provisional lorry entitlement to your licence. The D4 records a medical examination by a doctor. The doctor may ask an optician to complete the eyesight section. Use the current forms and instructions from DVLA.
You can order the application pack through GOV.UK. Do not confuse these forms with a booking form for practical lessons.
Before your medical appointment
Ask the medical provider what you need to bring. Check whether the appointment includes the eyesight section, what it costs and how payment works. Tell the provider about glasses or contact lenses and ask what medical information they need.
Take time to answer the medical questions accurately. If you are unsure about a health condition, discuss it with the doctor. UK HGV cannot decide whether you meet the medical standard.
After the appointment
Check that the required sections are complete before sending your application. Follow the current DVLA instructions for the documents to include and where to post them. A medical appointment does not itself give you provisional entitlement.
Keep a record of when you applied. Check your driving licence record before booking stages that require the new entitlement. Further medical enquiries can affect the time it takes.
Already had an HGV medical?
Tell your adviser when it happened and whether you have already sent an application. Ask what still needs checking before paying for another appointment. A previous medical does not automatically confirm that every requirement for your next course is complete.
